These terms and conditions govern the provision of chiropractic services ("Services") by The Mobile Chiropractor, a mobile chiropractic service provider ("Provider"), to the individual or entity receiving such Services ("Client").
1. Scope of Services:
1.1. Provider agrees to provide chiropractic services to the Client at locations designated by the Client within the agreed service area.
1.2. Services provided may include spinal adjustments, therapeutic exercises, soft tissue therapy, ergonomic advice, and other related treatments as deemed necessary by the Provider.
2. Fees and Payments:
2.1. Client agrees to pay the fees for Services rendered by the Provider as agreed upon prior to the appointment.
2.2. Payment shall be made in full at the time of service, unless otherwise agreed upon in writing by both parties.
2.3. Additional fees may be charged for travel outside of the standard service area, as agreed upon by both parties.
3. Appointments:
3.1. Appointments for Services shall be scheduled in advance by the Client.
3.2. Client agrees to provide accurate and current information for scheduling appointments.
3.3. Provider reserves the right to reschedule or cancel appointments due to unforeseen circumstances, with reasonable notice provided to the Client.
4. Client Responsibilities:
4.1. Client agrees to provide a suitable and safe environment for the provision of Services, including adequate space for treatment and access to necessary equipment.
4.2. Client agrees to disclose any medical conditions, injuries, or concerns that may affect the provision of Services.
4.3. Client acknowledges that the effectiveness of chiropractic treatment may vary depending on individual factors and commits to following any recommended treatment plans or exercises provided by the Provider.
5. Liability:
5.1. Provider shall not be liable for any injury, loss, or damage arising from the provision of Services, except in cases of gross negligence or willful misconduct.
5.2. Client acknowledges that chiropractic treatment involves inherent risks and releases the Provider from any liability arising from such risks, to the fullest extent permitted by law.
6. Confidentiality:
6.1. Provider agrees to maintain the confidentiality of all client information obtained during the provision of Services, in accordance with applicable privacy laws and regulations.
7. Termination:
7.1. Either party may terminate the provision of Services upon written notice to the other party.
7.2. In the event of termination, Client shall be responsible for payment for all Services rendered up to the date of termination.
8. Governing Law:
8.1. These terms and conditions shall be governed by and construed in accordance with the law in the state of Queensland, Australia.
9. Entire Agreement:
9.1. These terms and conditions constitute the entire agreement between the parties and supersede all prior agreements and understandings, whether written or oral, relating to the subject matter herein.
By engaging the services of the Provider, the Client acknowledges that they have read, understood, and agreed to these terms and conditions.
